How Can We Help?
If you are inducting a professional into the student or alumni chapter please use the Induction form and follow the instructions posted on the form.
If you are nominating a professional for the Eta Chapter please use the Professional nomination form (this form is only for Eta Chapter nominees). After the Professional nomination form is submitted please await confirmation that the professional has been invited to join. The professional or person of contact will then receive a payment form to complete the payment.
