How do I submit the Annual Chapter report?

Please use the following link to submit the Annual Chapter Report. Every Chapter is required to submit this report prior to June 30th. This form should be submitted after you have submitted your Chapter’s activities using the Chapter Activity Report.

Use this Quick Guide to help you through the steps of filling out the Annual Report.

To complete this report, you will need access to your current officer information. Ideally, this report should be submitted by a Chapter officer. In cases where a Chapter officer is unable to submit it, a Chapter advisor may submit the report.

Note: Chapter activity and annual reports will continue to be accepted for the 2023 – 2024 academic year through December 31, 2024. Chapters that have not yet submitted these reports should do so as soon as possible, and no later than December 31st.  Also, remember the Chapter Activity Report is a requirement for Key Chapter recognition.
