If you are inducting a professional into the student or alumni chapter please use the Induction form and follow the instructions posted on the form. If you are nominating a professional for the Eta chapter please use the Professional nomination form(this form is only for Eta chapter nominees). After the Professional nomination form is submitted please await confirmation that the professional has been invited to join. The professional or person of contact will then receive a payment form to complete the payment.

Please review the Chapter Status Page. On this page you can find your chapters reporting status over the past 2 years. This is especially useful for transitioning officers as they can determine the current status of the Chapter.

Please use the following link to submit the Annual Chapter Report. Every chapter is required to submit this annual chapter report prior to June 30th. This form should be submitted after you have submitted your chapter’s activities using the Chapter Activity Report.
Note: Chapter activity and annual reports will continue to be accepted for the 2018 – 2019 academic year through September 30, 2019. Chapters that have not yet submitted these reports should do so as soon as possible, and no later than September 30th.
To complete this report, you will need access to your current officer information. Ideally, this report should be submitted by a chapter officer. In cases where a chapter officer is unable to submit it, a chapter advisor may submit the report. Remember the Chapter Activities Report is a requirement for Key chapter recognition.
Please use the following link to submit the Chapter Activities Report after holding an event or program. Chapters should submit information about every meeting, activity, and event hosted or co-hosted after the meeting, activity, or event. Pro Tip: Complete a Chapter Activity Report with in two weeks after the activity to be able to capture as much detail as possible. These reports will be available to chapter officers, advisors and will be used to construct the annual chapter report. This way it is easier to submit the annual report since you have already compiled a list of events and activities throughout the year. For 2019 the Chapter Activities Report is also a requirement for the Key chapter Recognition.
Please fill out the Election of officers form to update or report new officers. This form allows us to keep track of chapter leaders and contact chapters on matters of IEEE-HKN. The form will require the names of chapter leaders as well as their IEEE numbers. If you do not have a chapter leaders IEEE number please reach out to them prior to submission. If a chapter leader cannot find or does not know their IEEE number please redirect them to the following link to the IEEE Support Center. For 2019 the Election of officers form is also a requirement for the Key chapter Recognition. Most chapters have the following positions:
